When Coverage Ends

e4 Health Employee Assistance Program coverage for you and your eligible dependents will end thirty days after the earliest of the following dates: 

  • the date that your employment with the Company ends for any reason, including layoff;
  • the date that you become part of a class of employees who are not eligible to participate in the plan;
  • the date that a dependent no longer meets the definition of an “eligible dependent” (see Eligibility and Plan Participation); or
  • the date on which the Company terminates the plan. 

If the e4 Health EAP is working with you on the effective date of any of the reasons stated above, your coverage will continue until your case is closed. 

The Administrative Information section of your Summary Plan Description (SPD) is incorporated into this section by reference. The Administrative Information section contains additional important information about your LifeScope Employee Assistance Program benefits, including your rights as a plan participant under the Employee Retirement Income Security Act of 1974 (ERISA).
