Filing Your Claim
You do not need to file a claim when you use a network attorney for covered in-office legal services. Your network attorney will file your claim for you.
WHEN TO FILE A CLAIM
You must file a claim if:
- you use a non-network attorney for a covered in-office legal service; or
- you use a reduced fee network attorney for a legal service that:
- is not listed as a covered legal service (see Covered Legal Services); and
- is not specifically excluded by the plan (see Exclusions and Limitations).
CLAIM FILING DEADLINE
If you use a non-network attorney for a covered legal service:
- you must notify the Claim Administrator within 60 days of contacting the non-network attorney; and
- you must file your claim within 120 days of the date that you incur an expense for the service.
The above deadlines also apply if you use a reduced fee network attorney for a service that is not listed as a covered legal service (and is not excluded by the plan).
CLAIM FILING PROCEDURE
You should follow these steps if you are required to file a plan claim:
Step 1: Obtain a claim form from the Claim Administrator.
Step 2: Complete the claim form and return it along with your attorney’s billing statement to the Claim Administrator.
Note: Your claim must be filed with the Claim Administrator within 120 days of the date that you incur the expense.

DENIED CLAIMS
You can appeal a denied claim. The claim appeal procedures are described in the Administrative Information section of your handbook.
