When Coverage Ends

This section describes the circumstances under which your Long-Term Care Insurance (LTC) plan coverage will be terminated. 

TERMINATION OF YOUR COVERAGE
Your LTC Plan coverage under this Policy will end on the earliest of the following dates: 

  • the date that your employment with the Company ends for any reason;
  • the date that you cease to be actively at work, unless you are on an approved leave of absence (as described in the Plan Participation section);
  • the date that you become part of a class of employees who are not eligible to participate in the plan;
  • the end of the period covered by your last premium contribution for LTC Plan insurance (or the end of any applicable grace period, if later) unless:
  • the date on which the plan’s total payments for covered services exceed your Certificate Limit;
  • the end of the month in which the Insurance Company receives your request for termination of plan coverage; or
  • the date on which the Company terminates the plan. 

TERMINATION OF ELIGIBLE DEPENDENT COVERAGE
LTC Plan coverage for an eligible dependent will end on the earliest of the following dates: 

  • the end of the period covered by the eligible dependent’s last premium contribution for LTC Plan insurance (or the end of any applicable grace period, if later) unless the dependent’s:
  • the date on which the plan’s total payments for covered services exceed the eligible dependent’s Certificate Limit; or
  • the end of the month in which the Insurance Company receives the eligible dependent’s request for termination of plan coverage. 

CONTINUATION OF COVERAGE (PORTABILITY)
You can continue (or “port”) your LTC Plan coverage if you retire or leave the Company for any other reason. Coverage for your eligible dependents may also continue, as long as they continue to pay the required premiums towards the cost of their coverage. 

If you elect to port your coverage, you will be billed directly or have your plan premiums deducted from your checking account. Your coverage will be continued at group rates,* as long as you pay your premiums when due and you have not exceeded your certificate limit. 

*The Insurance Company will notify you of any future changes in these rates
