How Your Vision Care Plan Works 

Your Vision Care Plan covers a wide range of vision care expenses. EyeMed Vision Care is the Claim Administrator for the Plan. 

EyeMed Vision Care maintains a nationwide network of participating vision care providers. Each time you or an eligible dependent needs vision care, you can choose: 

  • a network provider; or
  • an out-of-network provider. 

Covered Vision Care Plan expenses include: 

  • eyeglasses (lenses and frames)
  • contact lenses
  • laser vision correction (network providers only)

You can save time and money by using EyeMed Vision Care’s nationwide network of participating vision care providers, because: 

  • your network provider will file your claims for you; and
  • most network services are provided at a substantial discount (compared to the out-of-network cost).
You can locate participating EyeMed providers in your area by contacting EyeMed at 1-866-723-0596. You can also visit the EyeMed Vision Care website at and choose ACCESS from the provider locator dropdown box.

Your out-of-pocket costs may be higher if you use an out-of-network vision care provider (i.e., a provider that does not participate in the EyeMed Vision Care network). If you use an out-of-network provider, you will pay the difference between: 

  • the full amount of the out-of-network provider’s charge; and
  • the amount (if any) that EyeMed pays. 

If you use an out-of-network provider, you will also be responsible for filing your own claim. See the Filing Your Claim section for more information on filing out-of-network claims.
